• Collaborate with primary care providers in identifying and effectively managing mental health and psychosocial issues to improve patient outcomes as a valued member of the healthcare team.
• Conduct thorough patient assessments focusing on diagnostic and functional evaluations, providing actionable recommendations for treatment goals and plans to the primary care provider.
• Deliver concise, targeted therapeutic interventions tailored to the unique mental health needs of each patient.
• Guide primary care providers in determining the appropriateness of treatment for various patients, advising on referrals to specialized mental health resources as necessary.
• Initiate follow-up procedures to monitor patient progress, adjusting treatment plans as needed to promote optimal recovery.
• Develop and implement relapse prevention strategies, enabling patients to maintain stability in their mental health.
• Proactively identify at-risk patients, creating and executing plans to avert deterioration in their conditions.
• Coordinate healthcare services for behavioral health patients, connecting with external organizations as necessary while adhering to patient confidentiality.
• Maintain accurate records of patient progress and diagnostic information, ensuring compliance with establishment protocols.
• Communicate effectively with primary care providers regarding patient needs and progress, contributing to collaborative treatment planning.
• Facilitate behavioral changes in patients, particularly those facing physical illnesses, guiding them towards healthier lifestyle choices.
• Empower patients in their recovery by equipping them with self-management tools and educational materials.
• Consult with clinic management and team members on behavioral health services and engage in program evaluation discussions.
• Assist patients in adhering to treatments prescribed by their primary care providers, including addressing medication side effects and coping strategies.
• Uphold the Code of Ethical Conduct for Clinical Professionals, ensuring compliance and integrity in all aspects of your practice.
• Perform any other assigned duties that support the clinic’s mission.

Your work will profoundly impact a diverse patient population, primarily veterans and their families, addressing a range of mental health and psychosocial challenges. You will engage with clients of varying backgrounds, ensuring culturally competent care that meets the unique needs of individuals experiencing trauma, anxiety, depression, and other mental health issues.

• A minimum of a Master’s Degree in Clinical Counseling or a related field is required.
• Current LPC (Licensed Professional Counselor) credentials are essential.
• Comprehensive understanding of mental health assessment, treatment planning, and therapeutic methodologies, including family therapy, group therapy, child psychotherapy, and behavioral therapy.
• Familiarity with the Diagnostic and Statistical Manual of Mental Disorders (DSM-IV) for accurate diagnosis and planning.
• Excellent communication skills are paramount to effectively engage with patients, family members, and healthcare team colleagues.
• Demonstrated mental and physical health capacity to fulfill the role's demands.
• Awareness of the personal and social implications of various physical and mental health challenges on individuals, their families, and the community.

• Bi-lingual proficiency in English and Spanish is advantageous but not mandatory.
• Previous experience in telehealth services and familiarity with teletherapy platforms will be considered a plus.
• Established knowledge of community resources relevant to mental health services.
