Key Responsibilities
  • Conduct comprehensive psychosocial assessments to develop and implement individualized treatment plans tailored to each clientโ€™s unique needs.
  • Maintain thorough and accurate documentation of client interactions, treatment progress, and outcomes in accordance with department policies and regulatory requirements.
  • Actively participate in multidisciplinary team meetings to discuss client progress and adjust treatment plans as needed.

  • Serve as a point of contact for families and support systems, providing consultation and coordination regarding clientsโ€™ treatment and discharge plans.

Qualifications
  • A Master's degree in Social Work from an accredited institution.
