• Act as a liaison between clients, families, and outpatient providers to ensure a seamless continuum of care.
• Engage and collaborate with insurance companies on pre-certification processes and utilization management to facilitate client access to necessary services.
• Participate actively in developing discharge plans and aftercare strategies to support clients’ transitions from treatment back to their communities.
• Provide therapeutic meal support, guiding clients in developing a positive relationship with food, which is a vital part of their overall wellness.
• Deliver empathetic and affirming care tailored to the unique identities and circumstances of each client, promoting an understanding and collaborative therapeutic relationship.

In this role, you will primarily work with older adults and individuals facing complex mental health challenges. Your work will involve understanding their specific needs, preferences, and intersecting identities, thereby enabling you to provide personalized support that recognizes their unique life experiences. Clients come from diverse backgrounds, requiring you to adapt your approaches to best meet their therapeutic needs.

• Master's degree in Social Work from an accredited institution is required.
• A valid LCSW license in the state of Maryland is mandatory.
• Experience working with older adults is essential, as this demographic comprises a significant portion of our client base.
• Strong clinical and diagnostic skills with a proven track record in delivering psychological assessments and therapeutic interventions.
• Excellent communication and interpersonal skills to foster relationships with clients, families, and team members.
• The ability to work independently and effectively as part of a multidisciplinary team is critical.

• Familiarity with hospice care and end-of-life issues is beneficial but not required.
• Clinical experience in a remote work environment is an asset.
• Knowledge of insurance-related processes and experience with billing and coding can improve service delivery.
