• Conduct thorough psychosocial assessments to identify clients' mental health concerns, strengths, resources, and support systems.
• Deliver therapeutic interventions using individualized approaches, including individual, family, and group therapy sessions.
• Utilize evidence-based therapeutic modalities such as cognitive-behavioral therapy, solution-focused therapy, and trauma-informed care tailored to hospice settings.
• Design and implement personalized treatment plans that outline therapeutic goals, interventions, and measurable outcomes.
• Collaborate with multidisciplinary teams to ensure comprehensive and coordinated care that addresses all facets of client well-being.
• Provide crisis intervention services, conduct risk assessments, and engage in safety planning to support clients during episodes of acute distress.
• Maintain precise and timely documentation to adhere to ethical standards and regulatory compliance, ensuring continuity of care.

• A Master's degree in Social Work from an accredited institution.
• Active state licensure as a Licensed Clinical Social Worker (LCSW) in California.
