Prestige Family Healthcare Clinic, LTD, is seeking a dedicated and compassionate Licensed Mental Health Counselor (LMFT, LPC, LCSW) to join our integrated care team in Portland, Oregon. By merging mental health counseling with primary care, we ensure that our patients receive well-rounded and effective treatment plans, fostering their overall well-being and promoting recovery.

This position is fully remote, allowing for flexibility in your work-life balance while still engaging deeply with our patients and team. Our operational hours are Monday through Friday, from 8:00 AM to 5:00 PM, with occasional evening shifts based on patient demand. • Conduct comprehensive mental health assessments, evaluating the influence of medical conditions on patients' psychological well-being.

• Design and implement integrated treatment plans that address both mental health and medical requirements, ensuring a holistic approach to patient care.

• Facilitate individual and group therapy sessions using evidence-based practices tailored to the specific needs of each patient.

• Work closely with psychologists, psychiatrists, and other medical providers to deliver coordinated care, enhancing the effectiveness of treatment interventions.

• Provide crisis intervention and support during emergent mental health situations, ensuring the safety and well-being of patients.

• Educate patients and their families on the connection between mental health and physical health, empowering them to be active participants in their care.

• Maintain thorough and confidential documentation in compliance with all legal and ethical guidelines using electronic health record (EHR) systems.

• Engage in regular team meetings and case discussions to foster collaboration and shared learning among interdisciplinary team members.

• Continually stay informed on the latest research and advancements in integrated healthcare and mental health practices.

You will be serving a diverse patient population that includes individuals with co-occurring mental and medical health conditions. Your role will be crucial in addressing the unique challenges faced by these patients, who may experience chronic health issues that complicate their mental well-being. As a Mental Health Counselor, you will provide support, therapy, and education, guiding them towards greater stability and improved quality of life.

• A Master's or Doctoral degree in Social Work, Marriage and Family Therapy, Mental Health Counseling, or Psychology from an accredited institution.

• Current and valid licensure as an LMFT, LPC, or LCSW in the state of Oregon.

• Strong background in addressing the mental health needs of patients with concurrent medical conditions.

• Experience in delivering integrated care and collaborating effectively with multidisciplinary medical teams.

• Exceptional communication and interpersonal skills, capable of fostering trust and rapport with patients.

• Ability to maintain ethical standards and patient confidentiality at all times.

• Familiarity with electronic health record systems and proficiency in relevant computer applications.

• Strong teamwork skills and a commitment to collaborative care.

• A minimum of 1 year of experience in a mental health counseling role, preferably in an integrated care environment.

• Demonstrated ability to work with patients experiencing complex conditions, contributing to positive treatment outcomes.

This role offers a competitive salary range of USD 75,000 to 100,000 per year, complemented by an attractive benefits package.
