The work schedule comprises regular business hours during weekdays, with flexibility required to accommodate patient needs, which may include occasional evening or weekend hours.

• Conduct thorough psychosocial assessments through interviews, observations, and comprehensive reviews of medical records, evaluating emotional, social, and environmental factors affecting patient recovery.
• Provide one-on-one counseling and therapeutic support to patients and their families, addressing vital adjustment issues and fostering emotional resilience during this challenging time.
• Collaborate closely with a multidisciplinary rehabilitation team to craft individualized treatment plans that holistically incorporate patients' psychosocial needs and therapeutic goals.
• Assist patients and families in accessing relevant community resources such as support groups, vocational rehabilitation programs, and housing assistance, ensuring comprehensive care beyond the hospice environment.
• Deliver pertinent information and education to families regarding available community resources and post-rehabilitation services to facilitate smoother transitions to home or other care settings.
• Actively participate in discharge planning meetings, ensuring the smooth transition of care and supporting patients and families as they move towards recovery and re-establishing everyday life.

This role primarily serves patients in hospice care, including those facing significant end-of-life challenges. You will provide emotional and practical support to a diverse patient population, often involving individuals with chronic illnesses, terminal diagnoses, and varying degrees of psychosocial distress. Your work will have a profound impact on these patients' emotional landscapes and overall experiences as they navigate their treatment and care plans.

• A Master's degree in Social Work from an accredited program is required.
• Active state licensure as a Licensed Clinical Social Worker (LCSW) is essential to practice in this role.
• A strong understanding and experience in grief counseling, coping mechanisms, and adjustment issues are crucial.
• Exceptional communication and interpersonal skills are vital for building rapport and trust with patients and families.

• Experience in a rehabilitation or medical setting, particularly within a hospice environment, would be advantageous.
• Familiarity with evidence-based practices in social work and a solid grasp of community resource navigation is highly preferred.
