Passages Heart Diagnostic and Counseling Center is seeking a dedicated and experienced Licensed Mental Health Counselor to join our remote telehealth team. This full-time position involves providing high-quality mental health care primarily to the geriatric population, addressing various mental health issues with compassion and expertise. This position offers the flexibility of a remote work setting, allowing counselors to deliver care through telehealth services. Our hours of operation are typically Monday to Friday, from 9:00 AM to 5:00 PM, with the possibility of occasional evening shifts depending on patient needs.

• Conduct thorough mental health assessments tailored specifically for older adults, with an emphasis on understanding the multifaceted challenges they face.
• Develop and implement personalized treatment plans that consider cognitive health, physical conditions, and social factors affecting each patient.
• Utilize a variety of evidence-based therapeutic interventions, which may include individual and family therapy sessions, to treat conditions prevalent in the elderly.
• Work collaboratively with a team of geriatric psychiatrists and other healthcare providers to coordinate care effectively and holistically.
• Provide crisis intervention services during mental health emergencies, offering immediate support to elderly patients in distress.
• Educate patients and their families about mental health conditions, effective treatment options, and community resources, empowering them to make informed decisions.
• Maintain meticulous and confidential records in accordance with legal and ethical standards, utilizing our electronic health record (EHR) systems effectively.
• Participate actively in team meetings and case consultations to enhance care strategies and share insights on current practices in geriatric mental health.
• Continuously pursue professional development by staying updated on advancements in geriatric psychiatry and relevant therapeutic techniques.

As a Licensed Mental Health Counselor at Passages, you will primarily work with the geriatric population, focusing on those experiencing complex mental health issues such as depression, anxiety, dementia, and adjustment disorders. Your role will be pivotal in improving the mental well-being of elderly individuals, fostering an empathetic and supportive therapeutic environment that respects their unique life experiences.

• A Master’s or Doctoral degree in Social Work, Marriage and Family Therapy, Mental Health Counseling, or Psychology from an accredited institution.
• A valid and current license as a Licensed Clinical Social Worker (LCSW), Licensed Marriage and Family Therapist (LMFT), or Licensed Professional Clinical Counselor (LPCC) in California, or as a licensed psychologist.
• At least two years of post-licensure experience is preferred, demonstrating a strong foundation in mental health counseling, especially with elderly patients.
• Comprehensive knowledge of geriatric mental health disorders and familiar with evidence-based treatment modalities.
• Excellent interpersonal skills, characterized by a patient-centered approach that fosters trust and rapport with elderly clients and their families.
• Proficiency in navigating electronic health record (EHR) systems and other relevant digital resource management tools.
• A collaborative spirit suitable for working in a multidisciplinary team setting.

• Experience delivering therapeutic interventions and counseling techniques specifically geared towards older adults.
• Knowledge of community resources available for elderly clients, facilitating access to additional support networks.
• Adeptness in crisis intervention and management skills to effectively respond to emergencies involving elderly clients.

The annual salary for this role ranges from USD 115,000 to USD 130,000, commensurate with experience and qualifications.
