Key Responsibilities

• Conduct comprehensive assessments to gauge the emotional, psychological, and social needs of clients, including background history and individualized treatment goals.
• Facilitate individual, couples, and family therapy sessions using a variety of evidence-based therapeutic approaches, including but not limited to cognitive-behavioral therapy and mindfulness techniques.
• Develop and implement personalized treatment plans through collaborative discussions with clients, ensuring that interventions are precisely suited to their unique needs.
• Monitor client progress meticulously and adjust treatment strategies as needed to promote effective outcomes and healing.
• Adhere to professional ethics and confidentiality standards while documenting therapeutic interventions and client interactions accurately.
• Engage in ongoing professional development activities to remain informed of the latest advancements in social work practice and mental health care.
• Establish trust and rapport with clients, fostering a safe space for them to explore their thoughts and feelings.
• Provide practical strategies and emotional support for clients navigating the complexities of hospice situations, ensuring compassionate care at each step.

Patient Population / Client Focus

Your primary focus will be individuals and families who are attending hospice care, requiring sensitive, empathetic, and informed support as they cope with the challenges associated with end-of-life issues. The diverse range of clients includes those dealing with grief, loss, and emotional distress, necessitating a tailored approach to meet various individual needs.

Qualifications

• A Master’s degree in Social Work from an accredited institution.
• Active and valid state licensure as a Licensed Clinical Social Worker (LCSW).
• A minimum of two years of experience in a clinical or hospice setting, demonstrating an ability to work compassionately with clients facing serious illnesses and end-of-life challenges.
• Proficient assessment and treatment planning skills, alongside practical intervention implementation experience.
• Outstanding communication and interpersonal skills, underscored by a deep sense of empathy.

Preferred Experience

• Prior experience working in a hospice care environment or with terminally ill clients is highly desirable.
• Knowledge of grief and bereavement counseling practices.
