• Conduct comprehensive clinical assessments to evaluate patients' mental health needs and develop personalized treatment plans tailored to each individual's circumstances.
• Implement evidence-based therapeutic interventions, including counseling and psychotherapy, focused on alleviating emotional distress and enhancing quality of life.
• Facilitate regular virtual check-ins with patients and families, providing ongoing support and education about coping strategies, grief, and the dying process.
• Maintain accurate and timely clinical documentation, ensuring compliance with legal and ethical standards while minimizing paperwork to maximize direct patient care time.
• Collaborate effectively with a multidisciplinary treatment team consisting of physicians, nurses, chaplains, and other healthcare professionals to ensure holistic patient care.
• Keep abreast of the latest research and developments in hospice and palliative care as well as solution-based psychotherapy techniques to continually improve clinical practice.

The primary focus of this position will be on patients receiving hospice care, which includes managing chronic illnesses and supporting families as they face end-of-life issues. You will engage a diverse range of clients, from elderly patients to younger individuals facing terminal diagnoses, each requiring tailored support that respects their unique life situations.

• A Master’s degree in Social Work from an accredited institution is required.
• A valid and active LCSW license in the state of Pennsylvania is essential for this role.
• Demonstrable knowledge and experience with solution-based psychotherapy interventions, particularly in hospice or end-of-life care settings.
• Exceptional communication skills and the ability to work empathetically and effectively with patients and their families during difficult times.

• Prior experience in a hospice care setting is highly desirable, as it provides insights into the unique challenges and emotional needs of this patient population.
• Familiarity with virtual therapy platforms and electronic health record (EHR) systems such as Epic or Cerner for effective documentation and care coordination is an advantage.
