Job description

NurseCom is seeking experienced Medical Assistants to join its rapidly expanding Chronic Care Management program. In the role of Care Manager, you will bridge the gap between providers, specialists, and community resources to ensure continuity of care and resolve care barriers. Be a part of a dynamic remote Team of MA's lead by an RN under the supervision of physicians to conduct monthly calls to support your patients with complex chronic conditions, helping them manage their health and stay connected to their care team between regular office visits to achieve their maximum health potential.
Position
  • Minimum 35 hours/week
  • Fully remote (U.S. based only)
  • Minimum three years caring for patients with chronic conditions (diabetes, HTN, cardiology, primary care, etc.)

Responsibilities
  • Patient outreach to monitor overall condition, vital signs trends, and care plan progress
  • Ensure medication adherence, medication reconciliation, and identify refill needs
  • Support patients in care coordination, scheduling provider visits and preventative care appointments
  • Accurately document CCM encounters and time logs in the EMR to meet strict CMS billing requirements
  • Collaborate with providers and care team
  • Provide education to patients to understand and adhere to their chronic disease treatment plans and proactively engage in their wellbeing

Required Skills & Qualifications
  • Proof of active Medical Assistant certification (CCMA, CMA, RMA) from an accredited organization
  • Recent, three years in clinical setting, preferably in chronic care/remote care setting
  • Bilingual English/Spanish strongly preferred
  • Strong communication, organizational, and time management skills
  • Detail oriented with ability to work independently in a remote-team setting
  • Proficient with Word, PC laptop, and EMR systems (computer provided. Reliable, secure internet required
  • Private work-from-home office to manage patient protected health information
  • Passion for working with patients with chronic conditions and delivering exceptional care
  • Excellent clinical/customer care & phone communication skills. Patient-centric and empathetic bedside manner

Experience
  • Familiarity with remote care coordination tools and EMR-based task management
  • Ability to multi-task; prioritize and oversee multiple patient cases simultaneously
