Work Environment

This position is fully remote, allowing for flexibility in your work schedule. As part of a telehealth facility, you will utilize modern technology and electronic medical record (EMR) systems to connect with patients. Key Responsibilities
  • Conduct thorough assessments and evaluations of patients to address their unique mental health needs and develop appropriate treatment plans.
  • Foster effective communication among supervisors, direct care staff, families, guardians, and other relevant parties to coordinate care and monitor patient progress.
  • Provide structured scheduling of appointments, home visits, and follow-ups to ensure ongoing support and evaluation of treatment plans.
  • Coordinate in-home observations, staff interviews, and data analysis to understand patient environments and challenges better.
  • Ensure that program plans are implemented effectively by participating in interdisciplinary team meetings and updating treatment methods as necessary.
  • Maintain comprehensive documentation by completing monthly and quarterly progress notes, ensuring that records are readily accessible to involved parties.
  • Act as a liaison with the mental health community and the public to support patientsโ€™ developmental goals and promote mental health awareness.
  • Organize training programs to ensure staff competence in implementing treatment plans and maintaining proper documentation.
  • Provide continuous support through follow-up interviews and home visits, addressing patients' evolving needs.
Patient Population / Client Focus

Qualifications

  • Active and unrestricted licensure as a Professional Counselor (LPC) in the state of Arizona.

  • Strong understanding of legal and ethical standards in mental health practice.
  • Exceptional interpersonal and communication skills, with an ability to build rapport with clients from diverse backgrounds.

  • Familiarity with EMR software and telehealth technologies is necessary to manage documentation and patient engagement effectively.
Preferred Experience
