A remote clinical pharmacist role optimizing cardiac medication therapy and titration in partnership with nurses and cardiology providers.

Role Overview

Our client is seeking a full-time, contract Clinical Pharmacist with strong cardiology experience to support value-based care programs. This role focuses on optimizing guideline-directed medical therapy (GDMT), supporting medication titration, improving medication adherence, and collaborating closely with nurses and cardiology providers to improve outcomes for high-risk cardiac patients. This is not a dispensing role — the pharmacist functions as a clinical partner embedded within the care team.

Key Responsibilities

Medication Review & GDMT Optimization

  • Review patient medication profiles for cardiology populations — heart failure, coronary artery disease, atrial fibrillation, hypertension, and overlapping chronic conditions such as CKD.
  • Evaluate appropriateness of GDMT based on ACC/AHA guidelines and payer quality programs.
  • Identify gaps in therapy, contraindications, drug interactions, and adherence barriers.
  • Make evidence-based recommendations for medication initiation, optimization, and titration, in collaboration with providers and nursing staff.

Patient Engagement

  • Conduct telephonic medication reviews and patient education.
  • Educate patients on purpose, benefits, side effects, and expectations of cardiac medications.
  • Reinforce adherence and lifestyle considerations; help patients recognize red-flag symptoms.

Care Team Collaboration

  • Work closely with RN Care Managers and Nurse Navigators to align medication plans with care goals.
  • Communicate recommendations clearly to cardiologists and advanced practice providers.
  • Participate in interdisciplinary case reviews and care planning.
  • Document recommendations and patient interactions promptly in the care management platform / EMR.

Quality & Value-Based Care

  • Contribute to performance on medication-related quality measures.
  • Help reduce medication-related adverse events and prevent avoidable utilization.
  • Apply sound clinical judgment within established clinical protocols.

Who Will Succeed

A clinically confident cardiology pharmacist who is comfortable working telephonically, communicates complex clinical information in a patient-friendly way, and thrives inside a nurse-led, interdisciplinary care model.

  • Strong cardiology clinical background and comfort with GDMT decision-making.
  • Confident, professional communicator with cardiology providers and APPs.
  • Patient-centered communication style — clear, empathetic, and clinically accurate.
  • Comfortable in a fully remote, telephonic, non-dispensing clinical role.

Experience & Skills

Required:

  • PharmD
  • Active Texas pharmacist license in good standing
  • Multi-state pharmacist licensure required (remote role, not restricted to Texas residency)
  • Minimum 3–5 years of clinical experience with a strong cardiology focus
  • Demonstrated working knowledge of GDMT and cardiac medication titration
  • Comfort working within a nurse-led care management model
  • Strong telephonic patient communication skills
  • Ability to interact professionally and confidently with cardiology providers
  • Experience engaging directly with patients
  • Experience collaborating with physicians and advanced practice providers
  • Time zone compatibility — must be able to work during the client's required business hours - CT
  • Strong written and verbal communication — exceptional written English; assessed at screening

Preferred:

  • Prior experience in heart failure or cardiology clinics
  • Experience working within clinically integrated networks or value-based care models
  • Familiarity with ACC and AHA guidelines and payer quality programs
  • Experience documenting in electronic medical records or care management platforms
  • Prior remote or telehealth patient engagement experience
