• Conduct thorough psychosocial assessments to evaluate and address patients' emotional, social, and financial needs.
• Provide individual and group counseling, targeting various issues including grief, adjustment to recovery, and coping mechanisms.
• Coordinate care with multidisciplinary teams, including nurses, psychiatrists, and medical practitioners, to develop integrated treatment plans.
• Engage with community resources to facilitate post-discharge services, ensuring seamless continuity of care for our patients.
• Lead crisis intervention efforts, offering immediate support and advocacy during critical situations.
• Educate patients and their families about treatment options and available community support resources.
• Develop comprehensive discharge plans based on the individual needs of patients and connect them to appropriate resources.
• Participate actively in hospital committees to lend your expertise on social work-related matters, contributing to policy and procedure development.
• Maintain timely and accurate documentation aligned with hospital policies and regulatory standards.
• Foster a supportive environment, aiming to improve patient satisfaction and outcomes with each interaction.

In this role, you will work closely with individuals undergoing detoxification and recovery from substance use, as well as those facing significant mental health challenges. • Master's degree in Social Work from an accredited institution.
