This full-time position allows for a flexible schedule, encompassing day, evening, and weekend shifts based on departmental needs. You will be part of a dynamic hospice team that is driven by compassion and professionalism, working from a remote setting to meet the diverse needs of our patients. Your role will also involve on-call responsibilities, ensuring that you are available to provide support and intervention when necessary.

As a Licensed Clinical Social Worker, your daily responsibilities will include:

• Conducting detailed psychosocial assessments to understand patients' emotional, social, and financial needs.
• Collaborating with interdisciplinary medical teams to develop comprehensive care plans tailored to individual patient needs.
• Providing individual and group counseling to assist patients with adjustment issues, grief processing, and coping strategies.
• Coordinating with community organizations to facilitate post-discharge services and ensure smooth transitions in care.
• Offering emotional support and education to patients and families regarding treatment options and available resources, enhancing their understanding and engagement in care.
• Participating in hospital committees and contributing your expertise to social work policy and practice improvement.
• Utilizing crisis intervention techniques to provide immediate support to patients and families in distress.
• Developing and implementing discharge plans in conjunction with other healthcare professionals, connecting patients with vital community resources.
• Maintaining meticulous and timely documentation to uphold hospital policies and adhere to legal standards.

Your work will primarily focus on patients receiving hospice care, many of whom face complex emotional and social challenges during one of the most difficult times in their lives. You will be instrumental in helping patients and their families navigate these challenges, ensuring they receive the support needed to enhance their quality of life. This may involve addressing issues related to grief, end-of-life planning, and accessing supportive services in the community.

To be considered for this position, you must possess:

• A Master’s degree in Social Work from an accredited institution.
• An active state license as a Licensed Clinical Social Worker (LCSW).
• A comprehensive understanding of psychosocial assessment techniques and treatment modalities.

Though not mandatory, candidates with the following qualifications will be given preference:

• Two or more years of experience in a healthcare or hospital setting.
• A working knowledge of medical terminology and mental health disorders.
• Experience in crisis intervention and supportive counseling.
