Position Overview

Pro-Health Counseling Services is seeking a dedicated and skilled Licensed Independent Clinical Social Worker (LICSW) to join our team in a fully remote capacity. Key Responsibilities

• Conduct thorough behavioral health diagnostic evaluations and crisis assessments to determine the needs of patients in a hospice setting.
• Utilize standardized cognitive, behavioral, and mental health assessment tools to develop personalized care plans.
• Offer ongoing psychotherapy to both patients and their caregivers in individual and group settings, addressing their emotional and psychological needs.
• Coordinate care by actively collaborating with primary care providers, specialists, and home health aides to ensure cohesive support for the patient.
• Provide essential resources, guidance, and education to families and caregivers, equipping them to handle the complexities of hospice care.
• Deliver virtual support services, including teleconferencing and telephone consultations, ensuring accessibility for patients and families.
• Manage case activities for patients and their families, tracking progress and adjusting care plans as necessary.

Patient Population / Client Focus

Your focus will be primarily on adults facing serious illnesses, providing sociopsychological support that honors their unique journeys. Collaborating closely with multidisciplinary teams, you will help navigate challenging emotional landscapes, ensuring that patients and their families feel supported, understood, and valued during their time with us.

Qualifications

• A Master’s degree in Social Work from an accredited program and an active, unrestricted Licensed Independent Clinical Social Worker (LICSW) license valid in Washington State.
• A strong commitment to providing high-quality, patient-centered care in challenging environments.
• Proficient in assessment, diagnosis, and treatment in behavioral health, specifically tailored to hospice and palliative care settings.

Preferred Experience

• A minimum of three years of post-graduate experience in social work, ideally in hospice, palliative, or related healthcare environments.
• Demonstrated success in conducting individual and group therapy, with certifications in psychotherapy preferred.
• Experience working with interdisciplinary teams, with strong communication skills to effectively collaborate in patient care.
• Proficient in the use of telehealth platforms and electronic health records systems.

Compensation and Benefits

We offer a competitive salary ranging from USD 115,000 to 130,000 annually, complemented by a comprehensive benefits package. This includes:
