• Deliver effective clinical services through a range of interventions and therapies for individuals and families facing serious illness or bereavement.
• Utilize evidence-based treatment models to guide therapy sessions, ensuring compliance with CANS certification within two months of hire.
• Collaborate with healthcare teams and external organizations, such as schools and social services, to coordinate comprehensive care for patients and their families.
• Provide education and resources to empower clients to manage their health conditions effectively, including connection to community resources and support networks.
• Monitor and maintain compliance with Legal Rights Advisories (LRA), working with court personnel to facilitate necessary processes.

As a Clinical Social Worker at Unified Care NW, you will work primarily with terminally ill patients and their families, ensuring that their emotional, psychological, and social needs are met. You may also serve patients who are experiencing complex traumas related to their conditions, including grief and loss. Our approach centers on compassionate, patient-centric care that honors each individual's journey.

• Must hold a Bachelor’s degree in Social Work and possess a valid LICSW certification.
• At least three years of clinical experience in behavioral health or social work, preferably within hospice or palliative care settings.
• Specialized knowledge in integrating behavioral health within a primary care environment, with training provided as necessary.
• Experience working with diverse populations, demonstrating sensitivity and respect for various cultural and socio-economic backgrounds.

• Knowledge of Medicaid, Medicare, and private insurance plans, understanding the nuances and regulations associated with each.
• Experience with underserved and unhoused populations, including knowledge of their specific challenges and available resources.
