As a Licensed Professional Counselor, your key responsibilities will include:

• Conducting thorough patient intakes that involve diagnostic and functional evaluations, while providing actionable recommendations to primary care providers regarding treatment goals.
• Delivering focused mental health interventions to patients requiring immediate assistance in coping with psychological challenges.
• Collaborating with primary care providers to identify which patients can be served effectively within the primary care setting, and which patients necessitate referrals to specialty mental health facilities.
• Initiating follow-up care for patients to monitor their progress and adjust treatment plans when necessary.
• Developing relapse prevention strategies for patients to maintain stability and foster meaningful lifestyle changes.
• Identifying at-risk patients and collaborating in the creation of preventative care plans tailored to their needs.
• Overseeing the delivery of behavioral healthcare services, including coordination with external treatment providers with patient consent.
• Keen documentation of patient progress and diagnostic details in treatment records, while ensuring ongoing communication with primary care providers regarding patients’ treatment needs.
• Equipping patients with self-management skills and educational resources that empower them in their own treatment and recovery.
• Providing insights and consultation to clinic management and team members concerning behavioral services and potential areas for program evaluation.
• Supporting patients’ compliance with medical treatment prescribed by their primary care providers.
• Upholding the Code of Ethical Conduct for Clinical Professionals at all times.
• Performing additional duties as assigned by the management.

You will be working with a diverse patient population, addressing the mental health needs of individuals from various backgrounds and age groups. To be considered for this role, candidates must possess the following qualifications:

• A minimum of a Master’s Degree in Clinical Counseling.
• Valid LPC (Licensed Professional Counselor) license.

• Proficiency in English and Spanish is preferred, enabling effective communication with a wider patient demographic.
• A thorough understanding of the personal and social impacts of mental health issues on individuals and their families.
• Strong ability to form collaborative relationships with patients and professionals, ensuring continuity of care.
• Excellent communication skills, both written and verbal, to document care effectively and convey treatment plans.

While not required, ideal candidates may have experience in:

• Working in outpatient behavioral health settings.
• Utilizing specific therapeutic methodologies or evidence-based practices.
• Bi-lingual counseling or cultural competency training.
• Engaging with interdisciplinary teams to improve patient outcomes.
