Key Responsibilities

In this role, you will be responsible for:

• Providing educational and goal-oriented counseling to patients seeking mental health support, particularly those preparing for major life changes such as transplant processes.
• Assisting patients in navigating community resources, including home health services and vocational rehabilitation, to optimize their treatment outcomes and support their ongoing needs.
• Maintaining up-to-date knowledge of local vocational and educational programs, helping patients access necessary resources to facilitate employment or further education.
• Conducting thorough assessments of patients’ awareness of advance directives and facilitating discussions around this sensitive topic, ensuring their wishes are communicated to the healthcare team.
• Actively assessing psychosocial status as part of a comprehensive patient evaluation, identifying strengths and areas of need that may impact rehabilitation and treatment outcomes.
• Collaborating with patients and an interdisciplinary team to develop personalized care plans targeting rehabilitation goals and enhancing the patient's quality of life.
• Utilizing evidence-based patient education programs and quality of life assessment tools to address barriers and support the achievement of treatment objectives.
• Monitoring patient adjustment to care processes, particularly in complex cases such as dialysis, and providing interventions to support optimal psychosocial health.
• Delivering supportive counseling services within the scope of practice as determined by clinical training and licensing.

The patients you will be serving include a wide range of individuals seeking assistance in navigating mental health challenges, those managing chronic illnesses, and clients in need of support as they adjust to life-altering health decisions. You will work closely with patients of varying backgrounds, ages, and circumstances, tailoring your approach to meet their specific needs and promote healing.

Qualifications

The successful candidate will meet the following qualifications:

• Possession of a Master’s degree in Psychology, Counseling, Social Work, or related fields.
• Valid and current state license as a Licensed Mental Health Counselor (LMHC), Licensed Marriage and Family Therapist (LMFT), or Licensed Independent Clinical Social Worker (LICSW).
• Authorization to work in the United States.
• Clean history concerning medical malpractice claims, as verified by a third-party resource.

Preferred Experience

While not mandatory, experience in outpatient settings, familiarity with telehealth delivery systems, and knowledge of community resources related to rehabilitation would be advantageous. Candidates who possess strong communication skills and a history of advocating for patient-centered care will stand out.

Compensation and Benefits
