Key Responsibilities

As a Licensed Mental Health Counselor, your key responsibilities will include but are not limited to:

• Performing comprehensive patient assessments to evaluate psychosocial status, strengths, and areas of need that may influence rehabilitation outcomes.
• Delivering individualized counseling services that support patients in adjusting to treatment modalities such as dialysis, thereby enhancing their quality of life.
• Guiding patients seeking transplants through educational and goal-directed counseling that prepares them for the necessary lifestyle changes and emotional adjustments.
• Providing referrals to local community resources such as home health services and vocational rehabilitation programs, assisting patients in accessing these services as required.
• Facilitating discussions around advance directives with patients and their families when appropriate, ensuring that patients' wishes are respected and clearly communicated.
• Collaborating with healthcare teams to participate in care planning, identifying effective interventions that align with patients' rehabilitation and treatment goals.
• Utilizing established social work theories and quality of life measurement tools to analyze barriers and bolster patient engagement in their treatment process.

Patient Population / Client Focus

In this role, you will work with a diverse patient population primarily comprised of individuals undergoing dialysis and preparing for transplants. Your counseling will address both the emotional and practical implications of their health circumstances, guiding them toward improved psychological resilience and quality of life. Your engagement with patients will be crucial in understanding their unique challenges and helping them navigate through significant health transitions.

Qualifications

The ideal candidate will possess the following qualifications:

• A valid social work license as per state regulations (LMHC, LMFT, LICSW).
• A master's degree in Psychology, Counseling, Social Work, or a closely related discipline.
• No history of medical malpractice claims, verified by a third party.
• Authorization to work in the United States.

Preferred Experience

While not required, the following qualifications will be considered advantageous:

• Experience in outpatient mental health counseling or social work.
• Familiarity with treatment protocols for patients undergoing dialysis or seeking transplants.
• Proficiency in managing electronic health records and understanding telehealth platforms.

Compensation and Benefits
