Key Responsibilities

• Conduct thorough patient intakes focusing on diagnostic and functional evaluations, providing insight and recommendations to enhance treatment strategies.
• Assist primary care providers in recognizing and managing mental health and psychosocial issues as an integral part of the primary care team.
• Provide brief, targeted therapeutic interventions for individuals in need of mental health support.
• Advise healthcare colleagues on whether patients should continue in a primary care setting or be referred to specialized mental health facilities based on their specific needs.
• Initiate follow-ups to evaluate patient progress and determine if adjustments in treatment plans are necessary.
• Collaborate with a multidisciplinary team to develop personalized relapse prevention plans aimed at aiding patients in achieving and maintaining stable function.
• Monitor health service delivery related to behavioral health, ensuring fluid communication with other treatment providers.
• Document patient progress with accuracy in treatment charts, remaining compliant with privacy standards.
• Guide patients in self-management practices and inform them educationally to empower their treatment journeys.
• Provide consultations to management to enhance behavioral services and engage in program evaluation and outcomes improvement initiatives.
• Assist patients in implementing any medical treatments prescribed by primary care providers, including strategies to mitigate medication side effects.
• Uphold the Code of Ethical Conduct for Clinical Professionals and perform additional duties as assigned, adapting to the evolving needs of our patient population.

Qualifications

• A Master's Degree in Clinical Counseling is a minimum requirement.
• Possession of a current LPC (Licensed Professional Counselor) License.

• Familiarity with DSM-IV diagnoses and behavioral therapy modalities is essential.
• Abilities to effectively evaluate the needs of patients and establish robust relationships with providers, patients, and community resources.
• Strong clinical record-keeping and reporting skills are imperative for maintaining accurate patient information.
• Proficiency in English is required; bilingual skills in English and Spanish are preferable.
• Mental and physical health suitable to manage the demands of this position, along with the capability to exercise sound judgment in decision-making.

Preferred Experience

• Experience working within an outpatient facility or integrative care setting is highly desirable.
• Demonstrated knowledge of community resources and their application in mental health care.
• Prior involvement with evidence-based practices for treatment is advantageous.

Compensation and Benefits

Trinity Health Behavioral Center offers a competitive salary range of USD 87,000 – 105,000 annually, commensurate with experience and qualifications. In addition, hired candidates may be eligible for a SIGN-ON BONUS, highlighting our commitment to attracting qualified professionals.
