Harmony United Care Center is actively seeking a dedicated and compassionate Licensed Clinical Social Worker (LCSW) to join our team in a full-time remote role. This position is pivotal in delivering quality mental health services to patients at our Hospice Center in Chattanooga, Tennessee. This role is primarily remote, allowing for flexibility and the ability to serve clients in their own environments. While you will be part of a broad and supportive team, individual work with clients will occur through digital platforms, ensuring essential services are consistent and accessible. Collaboration with multidisciplinary teams, including physicians and nursing staff, reinforces our commitment to providing comprehensive care to each patient.

• Engaging with clients both in-office and virtually, performing thorough assessments for individuals experiencing major mental illness or severe emotional disorders.
• Collaborating as an integral team member, partaking in regular meetings and strategy sessions to enhance patient care and service delivery.
• Establishing rapport and trust with clients and colleagues, fostering an environment of respect and openness.
• Maintaining ethical standards by adhering to the guidelines set forth by relevant professional organizations and internal policies.
• Developing and implementing individual care plans, ensuring clients access necessary services and resources.
• Monitoring and documenting client progress, maintaining consistency in reporting and data entry to track improvement and outcomes.
• Assisting with crisis intervention when necessary, ensuring timely and effective response to urgent mental health needs.
• Participating in quality improvement initiatives within the organization to enhance service delivery and care outcomes.

The primary focus of this role is working with clients faced with severe emotional and psychological challenges who may come from varied backgrounds and experiences. You will be connecting with under-served populations, understanding their unique needs, and providing tailored support to improve their mental health and quality of life. • A valid LCSW license in Tennessee is required.
• Proven experience in clinical assessment, diagnosis, and treatment tailored to diverse populations.

• Exceptional organizational abilities with a capacity to manage multiple clients and tasks simultaneously.
• Demonstrated understanding of age-specific and developmental needs relevant to the populations served.

• At least 2 years of experience in a clinical setting, providing direct support and intervention for individuals with mental health concerns.
• Experience working with diverse populations in challenging settings, especially those related to hospice or end-of-life care.

This full-time position offers a competitive annual salary ranging from USD 70,000 to 85,000, commensurate with experience and qualifications.
