• Conduct comprehensive assessments to understand and address the needs of each patient, ensuring individualized care plans are developed and implemented effectively.
• Maintain clear communication with supervisors, staff, and family members to discuss patient progress and make necessary adjustments to treatment plans.
• Develop and maintain a meticulous schedule of appointments and home visits to monitor patient progress and engage in therapeutic interventions.
• Conduct in-home visits as needed to perform observations and participate in data collection, ensuring clients are receiving the appropriate level of support.
• Engage actively in interdisciplinary team meetings to coordinate care, share insights, and strategize on effective treatments for complex cases.
• Document and maintain comprehensive progress notes on a monthly and quarterly basis to ensure accurate reporting and evaluation of services provided.
• Facilitate training sessions for staff to ensure all team members are proficient in implementing program plans and understand documentation requirements.
• Serve as a liaison to the public and mental health community to enhance collaborative efforts that promote patient development and growth.
• Assist in the development of training materials and procedures to support continuous improvement of mental health programs.

The successful candidate will work with a diverse clientele, including both adults and youth dealing with a myriad of mental health challenges. Our services encompass a wide range of issues, such as anxiety, depression, trauma, and substance use disorders, ensuring counselors can utilize their skills across various therapeutic modalities. • A Master's or Doctoral degree in Counseling, Psychology, or a related field is required.
• Active and unrestricted licensure as a Professional Counselor (LPC) in Arizona.
• A minimum of two years of relevant clinical experience in a counseling role, providing direct support to clients.
• A strong understanding of legal and ethical standards applicable in mental health settings.
• Exceptional interpersonal communication skills to effectively establish rapport with clients and collaborate with team members.
• Proficiency in using Electronic Medical Records (EMR) systems and other relevant technologies to manage patient information efficiently.

• Specialization in areas such as family therapy, couples therapy, substance misuse treatment, or trauma therapy is highly preferred.
• Demonstrated experience in a similar clinical setting, with a focus on innovative approaches to patient care.
