• Conduct comprehensive assessments to determine the needs of patients, ensuring personalized treatment plans are established.
• Foster open communication between various stakeholders including supervisors, family members, guardians, and other professionals to monitor patient progress.
• Organize and submit a schedule of appointments, home visits, and observations to supervisors, allowing for effective monitoring of your practice and patient outcomes.
• Coordinate home visits to facilitate in-home observations, chart reviews, and data analyses to enhance treatment effectiveness.
• Collaborate closely with your team in interdisciplinary meetings, contributing to treatment decisions and ongoing care coordination.
• Document monthly and quarterly progress notes to maintain accurate patient records and support treatment planning.
• Lead training sessions for team members to ensure consistent implementation of clinical protocols and documentation standards.
• Provide continued support through follow-up interviews and home visits, catering to the evolving needs of clients.
• Act as a key liaison with the mental health community, advocating for patient needs and aligning development goals with community resources.
• Collaborate in the development of training materials aimed at enhancing program efficiencies and better serving our clients.

This role provides an opportunity to work with a diverse client base, including both adults and youth, who are facing a variety of mental health challenges. You will apply your skills in areas such as family therapy, couples therapy, and trauma-informed care to make a lasting impact on the lives of those you serve.

• A Master’s or Doctoral degree in Counseling, Psychology, or a related field is required.
• An active and unrestricted license as a Clinical Professional Counselor in the state of Illinois is essential.
• A minimum of 2 years of counseling experience, preferably within a clinical context.
• Strong knowledge of legal and ethical standards in counseling practice.
• Exceptional interpersonal skills, with the ability to establish rapport and trust with a variety of client demographics.
• Demonstrated ability to work collaboratively as part of a multidisciplinary healthcare team.
• Proficient in electronic medical record (EMR) software and relevant technologies to manage patient data and documentation.

• Specialization preferred in one or more of the following areas: family therapy, couples therapy, substance misuse, or trauma-focused therapy.
• Experience in outpatient settings utilizing telehealth platforms to conduct therapy sessions.

• This position offers a competitive salary range of USD 80,000 to 105,000 annually, commensurate with experience and caseload.
