• Engage with clients both over the phone and through virtual consultations, providing empathetic support and guidance.
• Conduct thorough assessments to gather essential information regarding clients' situations, crafting individualized treatment plans based on their unique needs.
• Facilitate access to community resources and services by making informed referrals to appropriate agencies.
• Develop and implement beneficial activities that encourage client engagement and participation in their care.
• Maintain accurate and comprehensive client records, ensuring all documentation meets legal and ethical standards.
• Recognize signs of child abuse and other critical issues, taking appropriate actions within the scope of your practice.
• Provide crisis intervention to clients in distress, employing your skills to de-escalate situations and offer support.
• Participate in weekly team meetings and ongoing professional development opportunities to enhance your skills.

You will be working primarily with clients dealing with urgent issues related to neglect, mental health, and familial challenges in a hospice environment. This role requires a deep understanding of the complexities involved in social work, particularly in sensitive contexts where clients are experiencing some of the most challenging times in their lives. Your ability to create a trusting rapport will be crucial in facilitating effective interventions and fostering a supportive environment.

• A Master’s degree in Social Work (MSW) or equivalent from an accredited institution.
• Current licensure as a Clinical Social Worker (LCSW) in Illinois.
• Strong proficiency with technology, including familiarity with Microsoft Office Suite (Word, Excel, Outlook, PowerPoint) and virtual communication tools.
• Exceptional communication skills, with the ability to listen actively and respond empathetically to clients’ needs.

• Experience working with clients affected by domestic violence, substance abuse, or mental health disorders.
• Demonstrated ability to develop crisis intervention strategies.
• Prior work in hospice or palliative care settings, understanding the unique needs of terminally ill patients and their families.
