  • Develop and implement personalized treatment plans based on the initial assessments, utilizing various therapeutic techniques including Cognitive Behavioral Therapy (CBT).
  • Maintain ongoing communication with clients regarding their progress, adjusting treatment plans as necessary and ensuring their engagement in the therapeutic process.
  • Collaborate with a multidisciplinary team during weekly meetings to discuss patient care strategies and provide insights gained from individual sessions.
  • Ensure timely and ethical completion of all documentation related to patient assessments and treatment plans, maintaining compliance with department standards.
  • Utilize case management techniques to connect patients with community resources, ensuring they receive holistic care that addresses physical, emotional, and social factors.
