The Core For Relationship Healing is seeking a dedicated and compassionate Licensed Professional Counselor (LPC) to join our team in Corpus Christi, Texas. Key Responsibilities

• Assist primary care providers in identifying, treating, and managing mental health and psychosocial issues, while actively contributing to the interdisciplinary primary care team.
• Conduct thorough patient intakes, performing diagnostic and functional evaluations to inform treatment goals and plans.
• Deliver brief, focused interventions to patients requiring immediate mental health services, adapting approaches to their unique needs.
• Provide expert recommendations to primary care providers, determining which patients would benefit from specialized mental health care versus those who can be effectively treated within the primary care setting.
• Initiate follow-up contacts to evaluate patient progress and make necessary adjustments to treatment strategies based on ongoing assessments.
• Develop relapse prevention plans for patients and support them in maintaining stable functioning in their daily lives.
• Identify at-risk patients and collaborate with healthcare providers to create comprehensive care plans aimed at preventing further deterioration of mental health conditions.
• Monitor and coordinate behavioral health services, facilitating connections with external treatment providers as authorized by patients.
• Document patient progress and diagnostic information accurately in treatment charts to ensure continuity and efficacy of care.
• Maintain transparent communication with primary care providers about patient needs and progress, contributing to a shared understanding of their treatment plans.
• Encourage patients to adopt healthier lifestyle choices and engage in self-management strategies that enhance their treatment and recovery.
• Consult with clinic management and interdisciplinary team members to evaluate the effectiveness of behavioral services and contribute to program development.
• Assist patients in adhering to medical treatments prescribed by primary care providers, including offering strategies for managing medication-related side effects.
• Follow the Code of Ethical Conduct for Clinical Professionals and carry out additional duties as assigned.

Patient Population / Client Focus

As an LPC at The Core For Relationship Healing, you will serve a diverse patient population with varying mental health needs, including individuals facing anxiety, depression, trauma, and interpersonal conflicts. The clinic's inclusive approach empowers patients by providing them with the education and tools necessary to participate actively in their own recovery journeys.

Qualifications

• A minimum of a Master’s Degree in Clinical Counseling or a closely related field.
• Current LPC (Licensed Professional Counselor) license for the state of Texas.
• Strong understanding of mental health assessment, treatment planning, DSM-IV diagnoses, as well as psychotherapeutic techniques.
• Exceptional communication skills, both written and verbal, with the ability to build rapport with patients and collaborate with other professionals effectively.
• Proficient in maintaining accurate clinical records and preparing detailed reports on patient progress.
• Ability to operate in a remote setting, demonstrating self-motivation and organizational skills.

Preferred Experience

• Bilingual proficiency in English and Spanish is preferred but not required.
• Demonstrated experience in a clinical counseling setting, with a focus on holistic approaches to mental health.
• In-depth knowledge of community resources, including referral services and support systems available for patients.
• Experience using telehealth platforms and tools to facilitate remote patient engagement.

Compensation and Benefits

This position offers a competitive salary range of $87,000 to $105,000 annually, commensurate with experience and qualifications.
