Job Title: Clinical Pharmacist


Location: 100% Remote (Candidates must hold a valid Florida Pharmacist License)
Contract Type: W-2 Contract
Work Schedule:

  • Flexible hours: Preferred working window is 10:00 AM to 6:00 PM EST or 11:00 AM to 7:00 PM EST

  • Part-time availability accepted

  • Weekend work optional

Position Overview:

Seeking a Clinical Pharmacist to provide Medication Therapy Management (MTM) services, focusing on Comprehensive Medication Reviews (CMRs) and promoting statin use per AHA guidelines. This role involves extensive outbound calling, clinical documentation, and transcription of medical and prescription data into a secure third-party system.

This role supports the Part D Star measures and contributes to member education and safety through clinical interventions.


Key Responsibilities:
  • Conduct 100 outbound calls per day to perform CMRs and MTM services

  • Document patient-specific prescription, diagnosis, and treatment recommendations in a third-party application

  • Deliver consultations based on AHA guidelines for statin use

  • Ensure high accuracy and speed in data transcription

  • Work independently while maintaining HIPAA compliance and quality standards

  • Participate in required training to maintain and improve performance


Qualifications:Licensing:
  • Active and unrestricted Florida State Pharmacist License is required

Education & Experience:
  • Minimum 4 years of experience as a Clinical MTM Pharmacist

  • Experience with Part D Star measures, particularly:

    • SUPD (Statin Use in Persons with Diabetes)

    • SPC (Statin Use in Persons with Cardiovascular Disease)

    • CMR (Comprehensive Medication Review)

  • Previous management experience is preferred

  • Experience in clinical medication reviews

  • Familiarity with pharmaceutical and medical terminology
