• Conduct thorough assessments of clients' mental health needs, developing tailored treatment plans that promote holistic growth and recovery.
• Provide individual, group, and family therapy services, utilizing evidence-based practices to engage clients effectively in their healing journey.
• Facilitate communication and collaboration among a multidisciplinary team, including medical professionals and community partners, to ensure comprehensive care for clients.
• Utilize technology platforms and electronic health record systems to document progress and outcomes efficiently, ensuring timely reporting of data.
• Participate in crisis intervention initiatives, providing immediate support and guidance to clients in distress.
• Deliver educational workshops and resources to the community, increasing awareness and understanding of mental health issues and available support.
• Monitor client progress through regular meetings, adhering to admission, discharge, and continuing care criteria.
• Maintain professional standards by adhering to ethical codes and the organizational policies set forth by Community Outreach Mental Care.

Your primary focus will be on vulnerable populations within the Stamford area, including individuals affected by socioeconomic challenges, diverse cultural backgrounds, and systemic barriers to healthcare. You will provide support for a wide range of mental health issues, including anxiety, depression, trauma, and severe emotional disturbances, ensuring that your interventions are both culturally sensitive and tailored to individual needs.

• A Bachelor's Degree in Social Work or a related field is required; a Master's Degree is preferred.
• Must possess a valid Licensed Clinical Social Worker (LCSW) credential in the state of Connecticut.
• A minimum of two years of direct clinical experience in a mental health setting is required.
• Strong knowledge of community resources and services, as well as familiarity with mental health laws and ethical practices.

• Experience working in remote and community settings, particularly with underserved populations.
• Comprehensive understanding of integrated care models and their application to mental health.
• Proficiency with electronic health records and other relevant software systems.

We offer a competitive salary ranging from USD 80,000 to 100,000 annually, commensurate with experience and qualifications.
