Work Environment

This remote position is designed to provide flexibility while still allowing our Social Workers to maintain a strong connection with their clients. Your daily work will primarily involve virtual meetings, telephone consultations, and online documentation, with occasional in-home visits when necessary. The role requires adaptability and strong time management skills, as the Social Worker will need to schedule appointments and maintain availability for urgent client needs at a variety of times, including possible evening or weekend hours.

Key Responsibilities
  • Conduct comprehensive assessments of seniorsโ€™ emotional, mental, and social health to identify their needs accurately.
  • Develop personalized care plans in collaboration with clients and their families, setting specific, achievable goals tailored to improve their quality of life.
  • Provide individual counseling sessions, offering emotional support and strategies for coping with transitions, loss, and other life challenges.
  • Advocate on behalf of clients, ensuring they have access to essential community resources such as healthcare, support groups, and social services.
  • Collaborate with healthcare providers, including nurses and therapists, to coordinate comprehensive care strategies that reflect our clientsโ€™ best interests.
  • Regularly monitor client progress, adjusting care plans as necessary and documenting all interactions clearly and accurately in our electronic health records system.
  • Educate seniors and their families about available resources, enhancing their ability to engage with the community and address their personal and social needs effectively.
  • Facilitate mediation and communication among family members to ensure that care strategies align with the clientโ€™s preferences and needs.
Patient Population / Client Focus

The primary population served includes seniors dealing with a variety of issues such as chronic health conditions, cognitive impairments, and social isolation. Qualifications
