This is a fully remote position, allowing you to work from the comfort of your home while remaining connected with our dedicated team. Our virtual environment is supported by advanced technology and a robust electronic health record (EHR) system that facilitates efficient communication, documentation, and client management. You will have access to resources that enhance service delivery and enable proactive engagement with clients. The typical work hours are Monday through Friday, 8:00 AM to 4:30 PM, with occasional weekend or evening hours as needed to meet the needs of the clients.

As a Licensed Clinical Social Worker, your key responsibilities will encompass a range of essential tasks aimed at enhancing the mental well-being of your clients:

• Conduct detailed assessments of clients' psychosocial needs, analyzing emotional, mental, and social factors that affect their lives.
• Collaborate closely with clients and their families to create personalized treatment plans tailored to individual goals, preferences, and health challenges.
• Provide expert counseling services, offering emotional support that empowers clients to navigate life transitions and overcome personal challenges.
• Act as a fierce advocate for clients' rights, helping them access vital community resources and ensuring they obtain necessary services.
• Work collaboratively with interdisciplinary teams, including healthcare professionals and caregivers, to coordinate care and optimize outcomes.
• Regularly visit clients virtually to monitor their well-being, assess living conditions, and address any emerging safety concerns.
• Facilitate effective communication among family members involved in the client's care, helping to manage conflicts and enhance support systems.
• Educate clients and their families about relevant community resources, support groups, and enriching programs that promote social engagement.
• Maintain accurate documentation including client assessments, treatment plans, and progress notes, ensuring compliance with all regulatory and ethical standards.

This role primarily focuses on serving a diverse client population, including individuals facing challenging mental health issues, seniors navigating aging-related concerns, and families in need of comprehensive psychosocial support. As an LCSW, your expertise will be instrumental in fostering resilience and encouraging well-being within this community. Understanding the unique challenges faced by these populations will enable you to make a meaningful difference in their lives.

• A Master’s degree in Social Work from an accredited institution.
• Current and valid licensure as a Licensed Clinical Social Worker (LCSW) in the state of Kansas.
• A minimum of two years of relevant experience in a mental health or social work setting, preferably within a community health framework.
• Possess a thorough understanding of social work principles, mental health challenges, and available community resources.
• Exceptional skills in assessment, counseling, and problem-solving.
• Ability to empathize and establish strong rapport with clients and their families.
• Strong written and verbal communication skills, conducive to effective teamwork and collaboration across various disciplines.
• Proficiency in using electronic health record systems and technology to enhance service delivery.

• Experience working in a remote setting or telehealth environment.
• History of effective collaboration with interdisciplinary teams.
• Knowledge of geriatric social work issues and treatment methodologies.
