Key Responsibilities
  • Conduct comprehensive biopsychosocial assessments tailored to the mental health, substance use, and medical needs of clients within the primary care framework.
  • Deliver high-quality behavioral health services, which include individual and family counseling, treatment planning, crisis intervention, and facilitating group therapy sessions.
  • Maintain accurate and electronic documentation of patient evaluations, progress notes, and crisis assessments, ensuring adherence to all relevant regulations and enhancing client care.
  • Act as a liaison to connect clients with local resources, providing assistance with social work services and referrals for medication management and community support programs.
  • Engage in collaborative efforts with Quality Improvement teams to design and implement data collection and analysis processes that enhance service delivery.
Patient Population / Client Focus

Your primary focus will be on individuals at various stages of substance use recovery, encountering unique challenges related to mental health and addiction. You will work with diverse populations, ensuring that counseling techniques and interventions are culturally sensitive and age-appropriate. Your role is pivotal in facilitating their recovery journey and promoting overall wellness.

Qualifications
  • A Master’s degree in Social Work from an accredited institution.
  • A minimum of three (3) years of experience as a licensed clinical social worker, with an emphasis on mental health and substance use.
  • Current state licensure as an LCSW is mandatory; candidates must stay compliant with continuing education requirements.
  • Proficient knowledge of crisis intervention strategies and counseling methodologies, enabling independent decision-making in therapeutic settings.
Preferred Experience
  • At least two (2) years of experience working in a mental health or clinical environment with a strong emphasis on individual and group therapy.
