Key Responsibilities
  • Conduct ongoing clinical assessments to evaluate the mental health needs of clients, ensuring their wellness goals are met.
  • Facilitate communication with multidisciplinary teams, including supervisors, direct care staff, family members, and other involved parties to discuss clients' progress and necessary adjustments to treatment plans.
  • Develop and maintain a tentative schedule of appointments, home visits, and observations to monitor clients' progress.
  • Coordinate and conduct home visits for thorough in-home assessments, compliance checks, and data collection relevant to client care.
  • Ensure the implementation of individualized program plans is both effective and aligns with established best practices.
  • Actively participate in Interdisciplinary Team Meetings to collaborate on patient care strategies and share insights with colleagues.
  • Document monthly and quarterly progress notes detailing client advancements and any necessary changes to their care plans.
  • Organize and lead training sessions for fellow staff to ensure competency in implementing program plans and maintaining accurate documentation.
  • Provide ongoing follow-up through interviews, home visits, and additional observations to ensure consistent support for clients.
  • Maintain thorough and confidential records to facilitate transparency and accessibility of information for relevant stakeholders.
  • Serve as a liaison for clients and the broader mental health community, fostering relationships that help clients progress in their treatment and personal goals.
  • Support the development of training materials and procedural guidelines to optimize program delivery for clients.
Patient Population / Client Focus

Our counseling services cater to a diverse clientele, including adults and youth from various backgrounds. This position allows you to work closely with individuals facing challenges such as family dynamics, substance misuse, trauma, and general mental health concerns. Qualifications
  • A Master’s or Doctoral degree in Counseling, Psychology, or a related field is required.
  • An active and unrestricted Professional Counselor license in the state of Arizona.
