The Cognitive Care Center is seeking a dedicated and compassionate Licensed Clinical Social Worker (LCSW) to take on a full-time remote role. As a Licensed Clinical Social Worker, you will be responsible for a variety of tasks crucial to the support and care of our clients:

• Conduct detailed psychosocial assessments of seniors, considering emotional, mental, and social facets to identify their needs.
• Collaborate closely with clients and their families to develop personalized care plans that align with their specific goals, preferences, and circumstances.
• Provide counseling and emotional support to seniors and their families, helping them navigate life transitions such as health challenges or loss.
• Advocate for clients, ensuring they have access to necessary community resources, support services, and programs to reinforce their well-being.
• Engage in regular check-ins and assessments to monitor clients’ well-being, identify any changes in their living conditions, and address concerns proactively.
• Facilitate communication among family members involved in care, aiding in conflict resolution and ensuring all parties are informed.
• Stay updated on current practices and regulations in social work to maintain high ethical standards in client interactions and documentation.
• Utilize electronic health record (EHR) systems to maintain accurate documentation of client assessments, progress notes, and care plans.

Your primary focus will be on the senior population, who often face complex health issues, psychosocial challenges, and transitions into new life stages. By utilizing your skills in assessment and intervention, you will work to ensure that seniors receive the holistic care they need, improving their quality of life and supporting their families through the process.

• A Master’s degree in Social Work from an accredited institution is required.
• Current and valid state licensure as a Licensed Clinical Social Worker (LCSW).
• A minimum of two years of experience in a social work role with a focus on senior care or community health settings.
• Comprehensive knowledge of social work principles, aging issues, and available community resources.
• Exceptional assessment, counseling, and problem-solving skills.
• Demonstrated ability to empathize, show compassion, and establish rapport with seniors and their families.
• Strong communication and interpersonal skills for effective collaboration with a diverse team.

While the minimum requirement is two years of relevant experience, candidates with experience in geriatric care, community health, or interdisciplinary care settings will be highly favored. Knowledge of telehealth practices and familiarity with EHR systems is a plus, as it will enhance communication and care coordination for our clients.
