Key Responsibilities
  • Act as a liaison between families and healthcare providers, ensuring clear communication regarding patient needs, preferences, and care plans.
  • Engage with insurance companies for pre-certification and utilization management, facilitating a smoother process for patient care coverage.
  • Participate actively in discharge and aftercare planning, coordinating follow-up services that align with both clinical goals and patient desires.
  • Provide therapeutic meal support, advising clients on establishing healthy relationships with food while modeling appropriate behaviors.
  • Deliver empathetic, affirming care, tailoring your approach to meet the diverse cultural and personal backgrounds of our clients.

Qualifications
  • A Master’s degree in Social Work from an accredited institution.
  • A valid LCSW license issued by the state of Georgia.
  • Strong clinical and diagnostic skills, particularly in working with older adults in hospice or similar settings.
  • Exceptional communication and interpersonal skills to engage effectively with patients, their families, and the multidisciplinary care team.

Compensation and Benefits

The position offers a competitive salary range of USD 90,000 to 104,000 annually, commensurate with experience and qualifications.
