Zenith Mind Counseling Center is seeking a dedicated Licensed Clinical Social Worker (LCSW) to join our team in a remote capacity, serving the needs of patients at our Hospice Center in Macon, Georgia. In this full-time role, you will provide compassionate support and intervention services to terminally ill patients and their families, helping them navigate the complexities of end-of-life care. This position offers a remote work arrangement, allowing you the flexibility to manage your professional responsibilities from the comfort of your home. Despite the remote setting, you will work closely with a collaborative team dedicated to hospice care, ensuring that communication and support remain strong. • Act as a liaison between families and healthcare providers, ensuring clear communication regarding patient needs, preferences, and care plans.
• Engage with insurance companies for pre-certification and utilization management, facilitating a smoother process for patient care coverage.
• Participate actively in discharge and aftercare planning, coordinating follow-up services that align with both clinical goals and patient desires.
• Provide therapeutic meal support, advising clients on establishing healthy relationships with food while modeling appropriate behaviors.
• Deliver empathetic, affirming care, tailoring your approach to meet the diverse cultural and personal backgrounds of our clients.
• Maintain accurate and confidential client records, using electronic health record (EHR) systems to track and manage patient information and care outcomes.

In this role, you will support a predominantly geriatric population facing end-of-life issues. You will work closely with patients who are grappling with complex emotions associated with terminal illness, as well as their families who may be navigating grief and loss. Your capabilities to address emotional, social, and psychological needs will be essential in contributing to each patient's care journey and overall quality of life.

• A Master’s degree in Social Work from an accredited institution.
• A valid LCSW license issued by the state of Georgia.
• Strong clinical and diagnostic skills, particularly in working with older adults in hospice or similar settings.
• Exceptional communication and interpersonal skills to engage effectively with patients, their families, and the multidisciplinary care team.
• Ability to work independently and collaboratively within a team-focused environment while demonstrating a strong sense of responsibility.

• Professional experience providing care in hospice, palliative care, or geriatric settings.
• Experience with mental health issues and a strong background in holistic approaches to patient care.
• Familiarity with EHR systems and other relevant healthcare technologies that enhance patient management.

The position offers a competitive salary range of USD 90,000 to 104,000 annually, commensurate with experience and qualifications.
