Job Description Bilingual Certified Nursing Assistant (CNA) Woodland Hills, CA Overview: Our company, specializing in Remote Patient Monitoring (RPM) and Chronic Care Management (CCM), is seeking a bilingual Certified Nursing Assistant (CNA) to support patient engagement and clinical care coordination. This role focuses on patient outreach, program participation, and collaboration with the medical team to help improve patient outcomes. Position Offers: Schedule: Monday-Friday, 8:30 AM-5:00 PM Competitive pay: $20-24/hour (DOE) Health insurance Paid Time Off (PTO) Sick Time Off (STO) Paid holidays Monthly productivity bonus Key Responsibilities: Conduct regular patient outreach for remote patient monitoring (RPM) and chronic care management (CCM) programs, including monthly outreach calls Triage patient needs and coordinate with providers, nurses, and clinical leadership Review remote monitoring data (blood pressure, glucose, heart rate) and escalate abnormal findings to clinical staff Manage an assigned patient panel, building strong ongoing relationships Document patient interactions, vitals, and updates in the EMR/EHR system, including uploading data into patient charts Provide patient education on care programs and available resources Monitor patient compliance and escalate non-compliance issues as needed Provide excellent patient service, empathy, and professionalism in all communications Qualifications: 2-5 years of experience as a Certified Nursing Assistant (CNA) or in a health customer service role, with strong tenure and experience managing a high call volume Bilingual in Spanish or Farsi Experience with vital sign readings or a similar clinical support environment strongly preferred Tech-savvy, independent self-starter with strong time management, computer skills, strong communication skills, and a genuine interest in managing chronic conditions Why Join Us: Join our growing healthcare organization dedicated to improving patient outcomes through remote monitoring and chronic care programs, with competitive pay, comprehensive benefits, and room to grow.
