This position operates in a remote setting, with a focus on collaboration and communication among a multidisciplinary team. You will join a group of healthcare professionals including psychiatrists, psychologists, nurses, and case managers who work together to provide integrated services. While working from home, you will utilize cutting-edge telehealth tools and electronic health records (EHR) systems to maintain organized and accessible patient information.

• Conduct thorough mental health evaluations, including psychosocial assessments and diagnostic interviews, to identify patient needs and challenges.
• Develop individualized treatment plans in close collaboration with patients, taking into account their specific goals, cultural background, and personal circumstances.
• Implement evidence-based therapeutic interventions such as cognitive-behavioral therapy (CBT) and solution-focused brief therapy (SFBT) across various environments including individual and group sessions.
• Provide crisis intervention when necessary, ensuring continuity of care for patients experiencing acute mental health distress.
• Accurately document clinical notes and treatment plans in conjunction with clinic policies to ensure compliance and quality care.
• Educate patients and families about mental health conditions, equipping them with strategies and resources for effective management.
• Regularly assess treatment efficacy through outcome assessments and modify treatment plans as needed to enhance patient progress.
• Stay up-to-date with advancements in mental health research and best practices, integrating this knowledge into everyday practice.
• Uphold ethical standards and ensure the confidentiality of patient information at all times.

In this role, you will primarily serve diverse populations within a community health clinic setting, reflecting a wide range of cultural backgrounds and mental health challenges. Our patient base includes individuals dealing with emotional distress, trauma, and various mental health disorders. • A Master’s degree in Social Work (MSW) is required.
• Must hold an active State license as a Licensed Clinical Social Worker (LCSW).
• Demonstrated ability to work independently and collaboratively, effectively communicating within a multidisciplinary framework.
• Exceptional interpersonal skills, with the capacity to build rapport with patients from diverse cultural and socioeconomic backgrounds.

• Experience in a community health clinic or similar healthcare environment is preferred.
