As a Licensed Clinical Social Worker at Embrace Life, your key responsibilities will include:

• Conducting thorough clinical assessments to develop personalized treatment plans that align with best practices in palliative care.
• Utilizing evidence-based therapeutic interventions such as Cognitive-Behavioral Therapy (CBT) and other modalities tailored to patient needs.
• Documenting patient progress and treatment plans in accordance with HIPAA regulations to ensure compliance and confidentiality.
• Working collaboratively with a team of psychiatrists, psychologists, and other specialists to provide cohesive care tailored to each patient's situation.
• Engaging in regular team meetings for case consultations to discuss challenges, share insights, and continuously improve service delivery.
• Educating patients and their families on available resources, coping strategies, and treatment options to empower them through the hospice experience.
• Staying informed about advancements in mental health practices to integrate the latest research into your work.
• Maintaining an active caseload while ensuring timely interventions to meet each patient's evolving needs.

Your focus will primarily be on patients in hospice care, many of whom are dealing with terminal illnesses and complex emotional and mental health challenges. You will support both patients and their families, addressing grief, anxiety, and end-of-life concerns while fostering a compassionate dialogue about their unique experiences and needs.

• A Master's degree in Social Work from an accredited institution.
• A valid Licensed Clinical Social Worker (LCSW) license in the state of Texas.
• A minimum of 3 years of experience in a clinical setting, with a focus on providing psychotherapy services.
• Strong foundation in solution-focused and evidence-based psychotherapy techniques.
• Exceptional communication and interpersonal skills, with a demonstrated ability to build trust with patients and families.
• Critical thinking and problem-solving aptitude to effectively address various patient needs.
• Proven ability to work successfully within a multidisciplinary team structure.

• Experience working within hospice or palliative care settings.
• Familiarity with community resources related to end-of-life care.
• Prior experience with or training in grief counseling and support.
• Ability to integrate spiritual care considerations into therapeutic practice.
