You will be actively communicating with healthcare professionals via telehealth platforms and collaborative tools, ensuring seamless delivery of care. • Conduct thorough assessments of clients’ emotional, social, and environmental needs to create personalized treatment plans.
• Collaborate effectively with the interdisciplinary team, including physicians, nurses, and other social workers, to ensure holistic care.
• Provide evidence-based therapeutic interventions, including individual, family, and group counseling tailored to patients and their families.
• Educate clients and their families about mental health resources, coping mechanisms, and community support services, enhancing their understanding of available options.
• Regularly review treatment plans and adjust interventions based on client progress, changes in conditions, or new assessments.
• Keep abreast of the latest advancements in mental health practices and hospice care to provide the best possible support for clients.

Our social workers play a crucial role in supporting patients who are facing terminal illnesses and their families. You will engage with clients of varying ages, cultural backgrounds, and health conditions, focusing on fostering resilience and connection during an emotionally taxing period. • A Master’s degree in Social Work (MSW) from an accredited program.
• An active Licensed Clinical Social Worker (LCSW) license in the state of Virginia.
• Proven ability to perform clinical assessments, evaluations, and develop comprehensive treatment plans.
• Exceptional communication skills, with a talent for building rapport and trust with clients and multidisciplinary teams.
• A strong ethical framework and commitment to confidentiality and sensitivity in dealing with complex personal situations.

• At least 2 years of experience in a clinical or hospice setting, involving direct interaction with patients dealing with grief, loss, and complicated emotions.
• Familiarity with remote counseling technologies and telehealth practices.
• Experience working with diverse populations and knowledge of community resources available for end-of-life care.

The annual salary for this position ranges from USD 80,000 to 90,000, commensurate with experience and qualifications.
