• Conduct detailed mental health assessments, including psychosocial evaluations and risk assessments, tailored to the unique circumstances of our hospice patients.
• Design and implement individualized treatment plans that consider each patient’s psychological, emotional, and social needs, while honoring cultural and personal values.
• Utilize evidence-based therapeutic interventions such as cognitive-behavioral therapy (CBT) and solution-focused brief therapy (SFBT) in both individual and family settings.
• Provide crisis intervention services, collaborating with interdisciplinary teams to ensure continuous care for patients in distress.
• Maintain accurate clinical documentation including progress notes and treatment summaries, complying with our clinic’s policies and regulatory standards.
• Educate patients and their families about mental health management, equipping them with coping strategies and resources to enhance their understanding and treatment adherence.
• Conduct regular outcome assessments to monitor treatment efficacy and make data-driven adjustments to care plans as necessary.
• Stay abreast of the latest research and best practices in mental health, integrating these findings into your therapeutic approach and organizational protocols.
• Uphold ethical standards and safeguard patient confidentiality in all aspects of professional practice.

The ideal candidate will be adept at working with a diverse patient demographic including individuals facing terminal illnesses, their caregivers, and families coping with bereavement. In this sensitive environment, you will address complex emotional experiences, providing holistic support that respects the personal values and cultural backgrounds of our patients.

• A Bachelor’s degree in Social Work is required, with a preference for candidates holding a Master’s degree in Social Work (MSW).
• Must possess a valid, active license as a Licensed Clinical Social Worker (LCSW) in the state of Texas.
• Demonstrated ability to work independently while effectively collaborating within a multidisciplinary team.
• Strong interpersonal and communication skills, enabling effective rapport-building with patients from various cultural backgrounds.

• Prior experience in hospice care or working with terminally ill patients is highly desirable.
• Experience with electronic health record (EHR) systems and telehealth platforms is a plus.
• Familiarity with evidence-based practices in mental health, including knowledge of trauma-informed care.

The position offers a competitive annual salary in the range of $70,000 to $84,000, commensurate with experience and qualifications. Additional benefits include:
