Position Overview

We are seeking a committed and compassionate Licensed Clinical Social Worker (LCSW) to join the team at Immersed Clinical Centre in El Paso, Texas. This full-time, remote position is dedicated to providing essential therapeutic support to patients and their families within our hospice care setting. Key Responsibilities
  • Conduct detailed mental health assessments, including psychosocial evaluations and risk assessments, tailored to the unique circumstances of our hospice patients.
  • Design and implement individualized treatment plans that consider each patientโ€™s psychological, emotional, and social needs, while honoring cultural and personal values.
  • Utilize evidence-based therapeutic interventions such as cognitive-behavioral therapy (CBT) and solution-focused brief therapy (SFBT) in both individual and family settings.
  • Provide crisis intervention services, collaborating with interdisciplinary teams to ensure continuous care for patients in distress.
  • Maintain accurate clinical documentation including progress notes and treatment summaries, complying with our clinicโ€™s policies and regulatory standards.
  • Educate patients and their families about mental health management, equipping them with coping strategies and resources to enhance their understanding and treatment adherence.
  • Conduct regular outcome assessments to monitor treatment efficacy and make data-driven adjustments to care plans as necessary.
  • Stay abreast of the latest research and best practices in mental health, integrating these findings into your therapeutic approach and organizational protocols.

Patient Population / Client Focus

Qualifications
  • A Bachelorโ€™s degree in Social Work is required, with a preference for candidates holding a Masterโ€™s degree in Social Work (MSW).

  • Demonstrated ability to work independently while effectively collaborating within a multidisciplinary team.
