• Conduct thorough assessments of patients to identify emotional, social, and environmental needs, fostering a clear understanding of their unique circumstances.
• Develop and implement individualized treatment plans based on evidence-based practices, ensuring alignment with each patient's care goals.
• Utilize therapy techniques such as solution-focused interventions, cognitive behavioral therapy, and grief counseling to help patients cope with their situations.
• Maintain accurate and confidential clinical records, adhering to relevant regulations to ensure compliance and quality of care.
• Collaborate with physicians, nurses, and other healthcare professionals within our multidisciplinary treatment team to provide holistic care.
• Offer emotional support and education to patients and their families, helping them navigate the complexities of the hospice experience.
• Stay engaged with current mental health research and continuously integrate new findings to enhance treatment approaches and patient outcomes.

This role is focused on supporting individuals facing life-limiting illnesses, as well as their families. The patient population is diverse, encompassing varying backgrounds and cultures, each with unique needs and challenges. Your work will greatly aid in alleviating emotional distress, managing symptoms of anxiety and depression, and fostering acceptance during end-of-life transitions.

• A Master's degree in Social Work from an accredited institution, ensuring a strong foundation in clinical practices.
• A valid LCSW-C license in the state of Maryland is mandatory to practice in our hospice environment.
• At least 2 years of clinical experience is required, providing a background in working with diverse populations and treatment modalities.
• Solid proficiency in solution-based psychotherapy interventions will be essential in delivering meaningful client outcomes.
• Effective communication and interpersonal skills are a must, as you will be working closely with patients, families, and interdisciplinary teams.

• Experience working within hospice or palliative care settings, understanding the specific challenges associated with end-of-life care.
• Familiarity with electronic health record (EHR) systems to manage patient data efficiently.
• A strong background in trauma-informed care practices and crisis intervention strategies.
