As a Licensed Clinical Social Worker, you will report directly to the Social Work Manager and undertake a variety of clinical tasks, including:

• Conducting thorough bio-psychosocial assessments to identify patients' psychosocial needs, barriers to treatment, and social determinants of health.
• Creating individualized treatment plans and applying evidence-based interventions through both individual and group therapy modalities.
• Collaborating with medical professionals to ensure comprehensive care and effective treatment strategies that enhance patient health outcomes and satisfaction.
• Documenting all social work activities accurately and timely in patients' medical records, adhering to departmental standards and the NASW Code of Ethics.
• Providing education and resources to patients and families to facilitate understanding of medical recommendations, supporting their journey toward optimal functioning.
• Engaging with community resources to advocate for patients and identify additional support systems that can enhance their social and emotional well-being.
• Maintaining knowledge of relevant local services and continually enhancing professional skills through appropriate educational activities and training.

Your expertise will be vital in providing care to a diverse patient population, including children, adolescents, and adults experiencing mental health challenges associated with acute and chronic conditions. In a hospice setting, you will work closely with families, addressing end-of-life issues, grief, and transition to ensure that both patients and their caregivers receive holistic support tailored to their unique situations.

To be successful in this role, candidates must meet the following qualifications:

• A Master’s degree in Social Work from an accredited graduate school.
• Active licensure as a Licensed Clinical Social Worker (LCSW) in accordance with Texas State Board of Social Work Licensure.
• Solid understanding of clinical social work practices, ethical standards, and the psychosocial aspects of patient care.

While clinical experience is essential, we prefer candidates who possess:

• At least 1+ years of relevant behavioral health experience, particularly in outpatient treatment settings.
• Demonstrated expertise in working with a variety of age groups and populations, particularly in hospice and palliative care environments.
• Familiarity with evidence-based therapeutic techniques and treatment modalities applicable to mental health issues.
