The Heritage Wellness Clinic is seeking a dedicated and compassionate Licensed Clinical Social Worker (LCSW) to join our remote team. Key Responsibilities
  • Conduct thorough assessments, utilizing diagnostic tools such as the Diagnostic and Statistical Manual of Mental Disorders (DSM), to formulate individualized care plans that respect cultural diversity and align with the unique needs of the population served.
  • Provide evidence-based therapeutic interventions, regularly reviewing and adjusting treatment strategies based on patient progress and feedback.
  • Maintain meticulous treatment records and clinical documentation using modern technology, ensuring compliance with regulatory requirements and organizational policies.
  • Collaborate closely with patients, their families, and interdisciplinary teams to create comprehensive treatment plans that outline measurable goals and desired outcomes.
  • Facilitate referrals to relevant external agencies or specialists, ensuring patients have access to a broad spectrum of support services.
