A typical day in remote chronic care management involves collaborating with patients over phone or video calls to assess their health status, reviewing medical records, developing and updating care plans, and coordinating with physicians or other providers as needed. You will spend a significant portion of your day monitoring patient progress, providing education on disease management, documenting interactions, and addressing any medication or care plan concerns. The role requires strong time management since you'll often be balancing multiple patient cases and tasks simultaneously.
