Stealth Agentic Home Care Operating Platform

The Opportunity
We are building a next-generation home care operating platform focused on creating a fundamentally better experience for patients, families, nurses, and caregivers.
Home care is a $130B+ industry, roughly half of which is private-duty care, yet the market remains highly fragmented and operationally inefficient. Families struggle to navigate care, caregivers often lack the training and support they need, and much of the industry continues to operate on legacy infrastructure and business models. We believe there is an emerging and rapidly growing demand for a higher quality level of at-home personal care services, and that now is the time to transform these services by empowering nurses to lead this care with a more clinical approach. Nurses have always understood best what a patient needs, and today's technology can enable them to steward caregivers to do better work than ever before.

25m Health is a healthtech venture studio built in partnership with LifePoint Health, Scion Health, and Apollo Global Management. We build healthcare companies from the ground up and invest in early-stage startups. Our platform marries an expansive clinical footprint and expertise with builders and operators to partner with aspiring entrepreneurs and startups to accelerate their path to market. 25m Health's in-house platform team, based in Nashville and New York City, spans engineering (full-stack, security, data science), strategy (fundraising, product, research), design (UI/UX, branding, marketing), and back-office (hiring, legal, accounting, tax).
We are seeking a Chief Nursing Officer (CNO) for our latest incubation focused on building a new model for home care. We're looking for a clinical leader with a strong point of view on what great home care looks like today, where the current model falls short, and how it should evolve. As Chief Nursing Officer, you'll work closely with the founding team to shape our clinical strategy and care model, define and measure what great care looks like, and bring the perspective of nurses, caregivers, patients, and families directly into how we build our technology, operations, and experience.
You will partner closely with the 25m Health team, experienced operators and executives with a track record of company creation, to refine the model, develop the go-to-market strategy, and launch the first markets. 25m Health will provide initial funding, shared resources, and ongoing strategic support as you scale the newly formed company.
What You'll Do:

  • Become a trusted voice in the home care nursing community: Build authentic relationships with exceptional nurses, grow awareness of the company, and inspire great clinicians to be part of what we're building.
  • Help nurses build something extraordinary: Partner closely with nurses as they step into leadership, build high-performing teams, reach more patients, and create meaningful impact in their communities.
  • Unlock new paths to patients: Build relationships with healthcare and community partners that help connect more patients and families with exceptional care.
  • Shape what we build for nurses: Bring the voice and expertise of nurses into everything we build, from delivering exceptional care to engaging families. Help translate the knowledge RNs bring to the home into products and experiences that make great care easier to deliver and scale.
  • Set a new bar for care: Define what exceptional clinical quality, patient experience, and frontline leadership look like and help make that standard consistent as the business grows.

As the company grows, you'll help us answer some of our most important questions. Where should we find high-quality nurses to join the platform? How do we help nurses become successful entrepreneurs? How do we create consistent standards of care while allowing nurses to build independent practices? Your experience and leadership will directly influence the kind of company and nursing community we become.

Compensation
  • You will work with us on a full-time basis for a period of 3-4 months before stepping into a leadership role in Newco.
  • $150,000-$175,000 annual salary, commensurate with experience (may vary based on experience, location, and other factors). This is an independent contractor engagement paid on a pro-rated basis.
  • Founder-level equity stake in the business.
