Work Environment

The remote work setting at Chat and Change Mental Health fosters flexibility and promotes a work-life balance that our employees value greatly. Your home office can become a space of productivity and comfort as you engage with clients through telehealth platforms. The structure of our work hours is Monday to Friday, from 9:00 AM to 5:00 PM, allowing for consistent interaction with patients and team collaborations throughout the week.

Key Responsibilities

• Provide individual and group psychotherapy to patients experiencing a range of mental health conditions, including anxiety, depression, and PTSD.
• Conduct comprehensive assessments of patients’ mental health status to gauge emotional, behavioral, and social functioning.
• Develop evidence-based treatment plans that are culturally competent and tailored to the unique needs of each patient, employing methodologies such as cognitive-behavioral therapy (CBT) or mindfulness-based interventions.
• Monitor patient progress regularly and make informed adjustments to treatment plans based on evolving needs and feedback from both patients and healthcare colleagues.
• Collaborate closely with psychiatrists, primary care physicians, and other healthcare professionals within our interdisciplinary team to maintain a holistic approach to patient care.
• Educate patients and their families about mental health conditions, treatment options, and available community resources through support groups and informational sessions.
• Adhere to ethical and professional standards of practice, ensuring the confidentiality and dignity of all patients by obtaining informed consent and maintaining accurate records.

Patient Population / Client Focus

Our patient population comprises individuals facing diverse mental health challenges, many of whom are also dealing with life-limiting illnesses. This role requires sensitivity to the complex emotional landscapes of patients and their families, emphasizing the creation of a safe and nurturing environment where they can explore their feelings, concerns, and coping strategies.

Qualifications

• Master's degree in Social Work from an accredited institution is mandatory.
• Possession of a valid and current state license as a Licensed Clinical Social Worker (LCSW).
• A minimum of 2 years of experience providing psychotherapy and mental health services in a clinical setting.
• Exceptional communication and interpersonal skills, enabling effective relationships with patients and collaboration within a healthcare team.
• Strong clinical skills, particularly in conducting thorough assessments and constructing evidence-based treatment plans.
• Experience with electronic health record systems (EHRs) and telehealth platforms to manage patient documentation and facilitate care coordination.
• In-depth understanding of various mental health conditions and empathy towards the challenges faced by patients.
• Commitment to maintaining patient confidentiality and adhering to ethical standards in practice.

Preferred Experience

• Experience in hospice or palliative care settings will be viewed favorably.
• A background in behavioral health interventions and crisis management techniques is a plus.
• Familiarity with community resources specific to the Babylon, NY area is beneficial for patient referral and support.

Compensation and Benefits
