Key Responsibilities

• Deliver high-quality psychotherapy and mental health services, utilizing evidence-based methodologies tailored to individual patients.
• Conduct comprehensive assessments of patients’ mental health status, evaluating emotional, behavioral, and social functioning to identify areas for intervention.
• Develop, implement, and regularly update personalized treatment plans that reflect culturally competent approaches and unique patient needs.
• Collaborate effectively with a multidisciplinary team, including psychiatrists, nurses, and primary care physicians, to ensure cohesive patient care.
• Monitor and assess patient progress, making timely adjustments to treatment plans based on patient feedback and clinical outcomes.
• Provide educational resources and emotional support to patients and their families, helping them understand mental health conditions and available treatments.
• Uphold the highest ethical standards by ensuring patient confidentiality and obtaining informed consent for all services provided.

Patient Population / Client Focus

As a Social Worker in our Hospice Center, you will primarily serve patients grappling with severe mental health conditions, including anxiety, depression, and post-traumatic stress disorder (PTSD). Your role will be crucial in supporting patients who often experience complex psychological challenges related to their illness. This position demands sensitivity to patients and family members dealing with end-of-life issues, requiring a compassionate approach and a deep understanding of grief and loss.

Qualifications

• A Master's degree in Social Work from an accredited institution.
• Current and valid state licensure as a Licensed Clinical Social Worker (LCSW).
• A minimum of two to three years of experience providing psychotherapy and mental health services.
• Demonstrated excellent communication and interpersonal skills for effective patient relationships and collaboration with healthcare teams.
• Strong clinical abilities, particularly in conducting assessments and formulating evidence-based treatment plans.
• Proficient in utilizing electronic health records (EHR) and other healthcare software for effective case management.

Compensation and Benefits
