Key Responsibilities

As a Licensed Professional Counselor at Invasive Care Consultants, your primary responsibilities will include:

  • Conducting thorough ongoing assessments to identify and address the needs of patients effectively.
  • Facilitating open communication among supervisors, direct care staff, family members, and other relevant individuals regarding patient progress.
  • Developing and providing a detailed schedule for appointments, home visits, and monitoring activities to present to supervisors.
  • Coordinating home visits and observations while conducting chart reviews and interviews to gather comprehensive data supporting client care.
  • Implementing and ensuring adherence to therapeutic program plans, monitoring effectiveness and relevance in service delivery.
  • Participating actively in Interdisciplinary Team Meetings to discuss patient care strategies and updates.
  • Completing meticulous monthly and quarterly progress notes to document patient advancements and therapeutic outcomes.
  • Providing follow-up support through home visits and continued engagement with clients to maintain therapeutic momentum.
  • Maintaining accurate and organized records to ensure information accessibility for all stakeholders involved in patient care.
  • Acting as a liaison within the public and mental health communities, fostering partnerships to enhance patient support systems.
  • Assisting in the development of training materials and procedures that elevate program efficacy and training compliance within the team.
Patient Population / Client Focus
