• Conducting thorough psychosocial assessments through in-depth interviews, observational evaluations, and medical record reviews to identify the emotional and social challenges impacting patients’ recoveries.
• Providing individualized counseling services to patients and their families, addressing a wide range of emotional issues including adjustment disorders, anxiety, and depression related to their health conditions.
• Collaborating with a multidisciplinary rehabilitation team to develop comprehensive treatment plans that reflect the psychosocial needs of each patient, ensuring holistic care.
• Facilitating access to essential community resources, including support groups, vocational rehabilitation, and transitional housing, empowering patients to manage their recovery actively.
• Offering educational resources and ongoing support to patients and their families about community resources and post-rehabilitation options to ensure continuity of care.
• Participating in discharge planning, coordinating with other healthcare professionals to ensure that patients receive seamless transitions back to home or alternative healthcare settings.

Your role will center around a diverse group of patients at our Detox Center who are tackling various challenges associated with substance use. You will engage with individuals from different backgrounds, each facing unique emotional hurdles as they embark on their recovery journey. Your understanding and sensitivity will significantly influence their ability to reintegrate successfully into their communities.

• A Master’s degree in Social Work from a recognized and accredited institution, providing a solid foundation in clinical social work principles.
• Active state licensure as a Licensed Clinical Social Worker (LCSW), affirming your qualifications to practice in Connecticut.
• Exceptional communication and interpersonal skills to establish rapport with patients and collaborate effectively with a diverse team of professionals.
• A profound understanding of common emotional and social adjustment issues, particularly those relevant to individuals recovering from substance use disorders.

• Previous experience in a rehabilitation or detox setting is highly preferred as it equips you with the necessary skills to address the specialized needs of this patient population.
• Experience in applying various therapeutic modalities and intervention techniques tailored to individual patient needs enhances the effectiveness of care delivered.
