Registered Nurse (RN) Case Manager - Remote - Oncology experience required

Work from home from anywhere in the U.S.! We are seeking a compassionate and clinically skilled RN Case Manager to join our team and help members navigate their healthcare journey through comprehensive case management and care coordination.

What You'll Do

  • Conduct telephonic and occasional face-to-face assessments to evaluate member healthcare needs.
  • Develop individualized care plans that improve health outcomes and overall wellness.
  • Coordinate services, resources, and programs to support members with complex medical and social needs.
  • Apply clinical expertise to identify risks, remove barriers to care, and facilitate successful transitions across the healthcare continuum.
  • Partner with multidisciplinary teams to optimize member outcomes and support effective claim management.
  • Engage members through strong communication and motivational interviewing techniques.

What We're Looking For

Required:

  • Active, unrestricted RN license in state of residence.
  • Willingness to obtain additional state licenses as needed (company-paid).
  • 3+ years of acute care hospital RN experience (medical-surgical, post-surgical, ICU, or similar).
  • 3-5-year acute oncology experience required
  • Associate Degree in Nursing (ADN).

Preferred:

  • Compact RN license.
  • Certified oncology nurse or certified chemotherapy nurse preferred
  • 1+ year of Case Management and/or Home Health experience.
  • Certified Case Manager (CCM) certification.
  • Proficiency with Microsoft Office (Word, Excel, PowerPoint).
  • Excellent telephonic communication and member engagement skills.
  • BSN preferred.

Schedule & Benefits

  • 100% Remote role; candidates may reside in any U.S. state.
  • Monday-Friday schedule with no weekends or holidays required.
  • Flexible 8-hour shifts between 8:00 AM-6:00 PM in your local time zone.
  • Occasional late-shift rotation until 8:00 PM.
  • Less than 5% travel for potential clinical audits.

The typical pay range for this role is:

$60,522.00 - $129,615.00

This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors. This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above.
