Job description

Immediate need for a talented Care Manager. This is a 06 Months Contract opportunity with long-term potential and is located in Florida (Remote). Please review the job description below and contact me ASAP if you are interested.

Pay Range: $33.50 - $37.10/hour. Employee benefits include, but are not limited to, health insurance (medical, dental, vision), 401(k) plan, and paid sick leave (depending on work location).

Key Responsibilities:


  • Day to day will include maintaining an active caseload of about 73 Behavioral Health Complex members at a time.
  • Outreaching member, primary care physician and outpatient providers on a biweekly basis
  • Initiating appropriate assessment using our internal EMR system
  • Creating a Care plan tailored to the member’s needs.
  • Updating Medications, diagnosis, and allergies in our EMR system on a minimum of a monthly basis.
  • Reviewing care gaps and offer education and resources in efforts to close those care gaps.
  • Connecting member to providers and resources in the community.
  • Attending scheduled community staffing and treatment teams as well as partnering with community agencies.
  • Staff complex members as needed with internal medical directors and clinical staff.
  • Shift: 8-5 Mon-Fri EST


Key Requirements and Technology Experience:


  • Skills-“Behavioral Health”, "Mental Health", "Social Worker" , "Mental Health Counselor", "LMHC" , "LCSW" , "LMFT"
  • Strong organizational and computer skills
  • Ability to work independently and collaboratively
  • Adapt to constant change with problem solving and a positive attitude
  • Required: Must have an active Florida Behavioral Health License (LMHC< LCSW< LMFT)\
